=== Metallurgical uses === Metal carbonyls are used in several industrial processes. Perhaps the earliest application was the extraction and purification of nickel via nickel tetracarbonyl by the Mond process (see also carbonyl metallurgy). === Contraindications === Treatment should be avoided in the presence of high fever or if the stool is bloody. Treatment is not recommended for people who could have negative effects from rebound constipation. If suspicion exists of diarrhea associated with organisms that can penetrate the intestinal walls, such as E. coli O157:H7 or Salmonella, loperamide is contraindicated as a primary treatment. Loperamide treatment is not used in symptomatic C. difficile infections, as it increases the risk of toxin retention and precipitation of toxic megacolon. Loperamide should be administered with caution to people with liver failure due to reduced first-pass metabolism. Additionally, caution should be used when treating people with advanced HIV/AIDS, as cases of both viral and bacterial toxic megacolon have been reported. If abdominal distension is noted, therapy with loperamide should be discontinued.
Because no free dextroamphetamine is present in lisdexamphetamine capsules, dextroamphetamine does not become available through mechanical manipulation, such as crushing or simple extraction. lisdexamphetamine is a single-enantiomer dextroamphetamine formula similar to Dexedrine but opposed to other amphetamine based pharmaceuticals such as Adderall, which contains both dextroamphetamine and levoamphetamine at a 3:1 ratio, or racemic mixtures like Evekeo and the presently discontinued Benzedrine which are amphetamine salts with a 1:1 enantiomer ratio. Studies conducted show that lisdexamphetamine may have less abuse potential than dextroamphetamine and an abuse profile similar to diethylpropion at dosages that are FDA-approved for treatment of ADHD, but still has a high abuse potential when this dosage is exceeded by over 100%.

Xanomeline/trospium chloride was approved for medical use in the United States in September 2024. It was the first antipsychotic to not act on D2 receptors. The mechanism of action instead relies on xanomeline's functional selectivity for the M1 and M4 muscarinic receptors, with trospium chloride, a peripherally selective antimuscarinic added to counteract xanomeline's unwanted peripheral muscarinic effects.
== History == Thiotepa and its synthesis were patented in 1952 by the American Cyanamid company. It was made for use in the textile industry and in the production process of plastics. However, thiotepa entered human trials in 1953 and was found to be effective against acute myeloid leukemia, chronic myelogenous leukemia, and Hodgkin's lymphoma. The first clinical trial noted a "reasonable margin for safety" between the apparent dose and undesired bone marrow suppression In January 2007, the European Medicines Agency (EMA) designated thiotepa as an orphan drug. In April 2007, the United States FDA designated thiotepa as a conditioning treatment for use prior to hematopoietic stem cell transplantation. In June 2024, the FDA approved a ready-to-dilute liquid formulation of thiotepa to treat breast and ovarian cancer.

== Interaction with Antibiotics == The ribosomal tunnel contains binding sites for clinically important antibiotics, such as macrolides or streptogramins. Antibiotics bind to the inner part of the ribosomal tunnel, either near the peptidyl transferase center or near the constriction site. Despite the original belief that macrolide binding to the tunnel causes obstruction, it has been shown that the tunnel remains permeable. Antibiotics instead stabilize the catalytic site of the ribosome in a conformation that is unproductive for peptide bond formation, causing translational arrest. Translational arrest of the ribosome may also be dependent on the sequence of the nascent peptide, as has been observed, for example with chloramphenicol.
=== Pharmacodynamics === Research has shown that salvinorin A is a potent κ-opioid receptor (KOR) agonist (Ki = 2.4 nM, EC50 = 1.8 nM). It has a high affinity for the receptor, indicated by the low dissociation constant of 1.0 nanomolar (nM). It shows atypical properties as an agonist of the KOR relative to other KOR agonists. In addition to its KOR agonism, salvinorin A has been found to act as a dopamine D2 receptor partial agonist, with an affinity of 5–10 nM, an intrinsic activity of 40–60%, and an EC50 of 48 nM. As such, the dopamine D2 receptor might also play a role in its effects. Salvinorin A has no action at the 5-HT2A serotonin receptor, the principal molecular target responsible for the actions of 'classical' psychedelics such as LSD and mescaline. Salvinorin A has also been shown to have effect on cannabinoid CB1 receptors. It significantly increases prolactin and inconsistently increases cortisol. It causes dysphoria by stopping release of dopamine in the striatum. Salvinorin A increases activity of DAT while decreasing activity of SERT. Salvinorin A is capable of inhibiting excess intestinal motility (e.g. diarrhea), through its potent κ-opioid-activating effects. The mechanism of action for salvinorin A on ileal tissue has been described as 'prejunctional', as it was able to modify electrically induced contractions, but not those of exogenous acetylcholine.
== Prognosis == As of 2015, 5-year survival rates for small cell lung cancer (extensive and limited) range between 3.6% and 32.2% for women, and between 2.2% and 24.5% for men. Relative 5-year survival rate for both sexes has increased from 3.6% in 1975 to 6.7% in 2014. In limited-stage disease, the relative 5-year survival rate (both sexes, all races, all ages) is 21.3%; however, women have higher 5-year survival rates, 26.9%, and men have lower survival rates, 21.3%. The prognosis is far grimmer in extensive-stage small-cell lung carcinoma where 5-year relative survival rate (both sexes, all races, all ages) is 2.8%; however, women have higher 5-year survival rates, 3.4%, and men have lower 5-year survival rates, 2.2%. Small-cell carcinoma is very responsive to chemotherapy and radiotherapy, and in particular, regimens based on platinum-containing agents. However, most people with the disease relapse, and median survival remains low. The overall incidence and mortality rates of SCLC in the United States have decreased during the past few decades. Long-term survival of more than 5 years can be achieved with proper treatment. According to the 17th World Conference on Lung Cancer (WCLC), "patients who received chest radiation and prophylactic cranial irradiation along with a mean of five chemotherapy cycles could achieve a median survival of more than 5 years." In some cases, long-term survival of 10+ years is achieved with chemotherapy and radiation alone. A 2023 article stated that the median overall survival is about 1 year, the worst of any lung cancer subtype.
== Mental health assistance == Due to the discrimination they face, trans individuals often present significant mental health disparities. Among them, it is estimated that 32-50% of trans individuals across various countries have attempted suicide, with this high prevalence attributed to factors such as victimization, bullying, violence, social and familial rejection, as well as discrimination in different public sectors. Other than gender dysphoria, trans individuals are also affected by mental health diagnoses, such as major depression and generalized anxiety, at far greater rates than the non-trans population. Whilst gender-affirming care has a positive impact on trans individuals' mental health and may lessen several symptoms, the psychological improvements of such care are often limited by the minority stressors affecting the trans population. Due to these factors, mental health assistance through means such as psychotherapy may play an important role in addressing trans patients' mental health. Mental health assessments and treatment may be conducted prior to the initiation of gender-affirming care as a means of ensuring the patient's informed consent and complementing medical transition. Pyschotherapy as a requirement for the inititation of affirmative medical care today is discouraged by trans health care protocols, as are approaches attempting to change the patient's identity or skew self-exploration towards one particular direction.
== Career == After her PhD, Chutia continued her research at Dibrugarh University for another year as a CSIR-postdoctoral fellow. She entered the field of plasma physics at the Physical Research Laboratory in Ahmedabad and then joined the Institute for Plasma Research in Gandhinagar. She then returned to the Institute of Advanced Study in Science and Technology as a faculty member and set up the Plasma Physics Laboratory. After finishing a fellowship awarded by the Japanese government in 1988 to work at the Plasma Laboratory of the Institute of Space and Astronautical Science, Tokyo, in 2005 she became the director of the Institute of Advanced Study in Science and Technology.
